Trevor Reznik, an insomniac machinist at a factory, hasn't slept in over a year due to extreme insomnia, causing his body to waste away. He's obsessed with tracking his weight loss on sticky notes and has few human connections aside from Stevie, a prostitute he pays for emotional companionship. Trevor is infatuated with Maria, a waitress he sees at an airport diner but never interacts with. His colleagues distrust him due to his emaciated appearance and odd behavior. After a workplace incident further isolates him, Trevor finds strange notes in his apartment and believes they're from Ivan, a phantom employee who doesn't exist according to his coworkers. Driven by paranoia, Trevor searches for evidence of Ivan's schemes while trying to uncover the cause of his insomnia.
